***COVID VACCINE UPDATE***

Please be assured that everyone who needs the vaccine will be offered it. The NHS will be in touch with you when it is your turn to be vaccinated. Please don’t call us before you are invited. In the meantime, you can get information about the COVID-19 Vaccination online at www.nhs.uk/covidvaccination

  • If you are visiting Cornwall and need to see a doctor, please contact your own GP practice. Practices throughout the country are offering telephone and video appointments and can electronically send prescriptions to any local pharmacy here for you to collect.

New Patient Registration

westoverIf you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill along with one form of photo ID.

You will need to complete a registration form (GMS1) which you can print below using the link below or get from reception.  The form needs to be given to the receptionist who will give you a health questionnaire  to complete.  The health questionnaire will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

All new patients who have a chronic disease, such as asthma, diabetes, COPD and hypertension are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.
